Please add remarks here: ************************************************************************ NOTE: Please have the person who actually designed the network complete the form and submit it to Linda Werner via e-mail linda@sat-tel.com. SCSI’S IP NETWORKS ARE NON-PORTABLE. If you cancel your Internet circuit you may not transfer SCSI IP networks to another provider. ************************************************************************ Please complete all the fields below, unless otherwise indicated. Submission of this form indicates agreement of SCSI’s acceptable use policy. This policy can be found on our web site www.sat-tel.com. ************************************************************************ 1. Customer SCSI Order Information ---------------------------------- Provide the information about the customers SCSI order. If the customer has more than one circuit, you must indicate which circuit the networks will be used on. SCSI cannot issue IP address space for IP networks that arent connected to the Internet. ------------------------------------------------------------------------ Company Name : Is the company an ISP (Y/N): Company Street Address : : : SCSI Customer? (Y/N) : Circuit ID # : ----------------------------------------------------------------------- Is company multihomed? (Y/N) : If so, list how (circuit : ids, other providers, etc) : : ------------------------------------------------------------------------ ************************************************************************ 2. SCSI Point-of-Contact ------------------------ Provide the information below for the SCSI Point-of-Contact (POC) making the IP Network request for the customer. ------------------------------------------------------------------------ SCSI Contact First Name : SCSI Contact Last Name : SCSI Phone : SCSI E-Mail Address : ------------------------------------------------------------------------ ************************************************************************ 3. Customer Point-of-Contact ----------------------------- Provide the information below for the customers technical Point-of-Contact (POC). This is the POC for resolving technical problems associated with the network and for updating information about the network. The technical POC may also be responsible for hosts attached to this network. ------------------------------------------------------------------------ Technical POC First Name : Technical POC Last Name : POC Phone : POC E-Mail Address (required) : ------------------------------------------------------------------------ ************************************************************************ 4. Size of Network Request --------------------------- In "Number of addresses requested", put the size of the network block you are requesting. For example, this could be: 1 class C network /21 aggregate block In "Number of hosts in the network", put the number of IP addresses that will be used by equipment in your network in the next 3 months. This number is NOT the amount of available address space in the IP network block. ------------------------------------------------------------------------ Number of addresses requested: Number of hosts in the network: ------------------------------------------------------------------------ ************************************************************************ 5. Justification ----------------- ------------------------------------------------------------------------ ------------------------------------------------------------------------ The more numbers you are requesting, the more detailed your technical description will need to be. Please include details of your network deployment plans, describing the network now (to next 3 months) and in one and two years. Please only request what you need for the next 3 months. You can ask for more networks when you grow. In addition, please complete the table in this section which asks for details of your addressing plan. Specifically you will need to state per subnet, the subnet mask, the number of hosts now (to next 3 months) and those planned for one and two years time. Please add any explanatory remarks you feel necessary. If you are asking for any networks for future growth, please include the word "growth" in the Remarks column. If you are an ISP, indicate which networks are for your network and which networks are for your customers in the remarks field. You do not have to list the customers name if you want to protect them, you can instead put "customer 1", "customer 2", etc. If you are asking any networks for future customers, please include the word "growth" in the Remarks column. New ISPs are required to adhere to the InterNICs slow-start policy of requesting small blocks of address space, using it up, and then coming back for more. At this moment in time, address space is a limited resource and we are doing our best to ensure that it is used in an efficient and responsible way. SCSI follows the InterNIC policies for conserving IP address space. *** Example: *** Number of hosts per subnet for *** Net/Subnet Subnet mask Max 0-3Mo Year+1 Year+2 Remarks ------------------------------------------------------------------------ 1.0 255.255.255.192 62 20 35 55 Backbone 1.1 255.255.255.192 62 16 28 48 Production Lab 2.0 255.255.255.224 30 5 15 25 Backbone 2.1 255.255.255.224 30 16 25 30 Production Lab 2.2 255.255.255.224 30 8 15 25 Backbone 2.3 255.255.255.224 30 12 20 30 Production Lab 2.4 255.255.255.224 30 13 15 25 Production Lab 2.5 255.255.255.224 30 16 28 30 Production Lab 3.0 255.255.255.0 254 200 225 250 Main LAN *** The following table will help you with subnetting: *** Subnet Mask Aggregate #Subnets #Hosts per subnet ------------------------------------------------------------- 255.255.255.0 /24 1 254 255.255.255.192 /26 2 62 255.255.255.224 /27 6 30 255.255.255.240 /28 14 14 255.255.255.248 /29 30 6 255.255.255.252 /30 62 2 ------------------------------------------------------------------------ Expansion Network Details : Number of hosts per subnet for Net/Subnet Subnet mask Max 0-3Mo Year+1 Year+2 Remarks ------------------------------------------------------------------------ ----------------------------------------------------------------------- ************************************************************************ *** 6.
